📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people in their homes to help bridge gaps in care and improve health outcomes • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Make referrals to the individual’s health plan and encourage participation of loved ones • Ensure primary care physicians are aware of patients’ health and quality of life

🎯 Requirements

• Physician credential as MD or DO • Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably work in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants • Multi-state licenses preferred • Ability to provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
