📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with members one-on-one and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Make referrals to health plans and encourage loved ones' participation • Ensure primary care physicians are aware of members' health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes containing household pets, secondhand smoke, and other potential airborne allergens or irritants • Multi-state licenses preferred
