📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Answer health questions and help identify gaps in care • Make referrals to the individual’s health plan • Encourage participation of members’ loved ones • Ensure primary care physicians are aware of members’ health and quality of life • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Provide Signify Health Diagnostic & Preventive Services products

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Multi-state licenses preferred
