📋 Description

• Provide in-home health evaluations in members’ residences • Engage with members and build trusted relationships • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Answer health questions and help bridge gaps in care • Use an iPad and clinical workflow for each evaluation • Provide education and connections to appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services • Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights • Visit members nationwide as part of a clinician network

🎯 Requirements

• Primary residency in Connecticut • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within Connecticut and flex to other areas in cases of business need • Able to travel locally and within licensed states, up to 100% of the time • Able to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Ability to conduct virtual visits as needed • Ability to provide Diagnostic & Preventive Services, including spirometry and diabetic eye exams • Ability to use an iPad and clinical workflow • 40 hours per week
