📋 Description

• Design, build, and maintain Healthy Planet registries, care plans, care teams, and health maintenance topics • Configure and optimize risk stratification tools, care gap identification, and outreach workflows within Healthy Planet • Lead testing and validation of Healthy Planet builds, including registry logic, care plan triggers, and reporting outputs • Design and maintain care coordination workflows for transitions of care, discharge planning, referral management, and care team communication • Optimize Care Everywhere and interoperability workflows for information exchange across care settings • Partner with care management and case management teams to streamline documentation, task management, and communication tools within Epic • Serve as the primary Epic liaison for Population Health, Care Management, and ACO stakeholders • Translate program goals into technical requirements • Facilitate workflow design sessions with clinical and operational leadership • Present build recommendations, timelines, and trade-offs to leadership and governance committees • Mentor junior and mid-level analysts on build standards, testing practices, and troubleshooting • Review and validate build work completed by team members before release • Contribute to documentation, standard operating procedures, and onboarding materials • Partner with reporting teams to support dashboards and quality/outcomes reporting • Participate in change control, release planning, and regression testing • Maintain documentation of workflows, build configurations, and design decisions • Manage and groom user stories and backlog items in agile tools such as Rally

🎯 Requirements

• Bachelor's degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience) • 4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules • Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred • Demonstrated experience designing workflows for population health, care management, or value-based care programs • Strong understanding of transitions-of-care processes and cross-continuum care delivery • Experience in an ACO, health system, or integrated delivery network with population health responsibilities preferred • Familiarity with FHIR/HL7 interoperability standards and Care Everywhere configuration preferred • Experience mentoring or informally leading junior analysts preferred • Familiarity with agile delivery tools such as Rally for backlog and release management preferred • Deep Epic build expertise • Ability to translate clinical/operational goals into scalable technical solutions • Strong stakeholder communication across clinical, technical, and leadership audiences • Comfortable balancing hands-on build work with cross-functional project leadership
