📋 Description

• Conduct outbound member outreach and engagement activities. • Support scheduling of assessments and care management appointments. • Process referrals and assist with care management enrollment activities. • Administer questionnaires and health risk assessments as directed. • Provide appointment reminders and post-appointment follow-up. • Document activities and enter data into care management systems. • Respond to phone queue inquiries and route issues appropriately. • Support care managers with administrative and non-clinical coordination activities. • Assist with authorization-related tasks and other operational workflows.

🎯 Requirements

• High School Diploma or GED required. • Strong communication, customer service, and organizational skills. • Ability to work effectively in a fast-paced care management environment. • Strong verbal and written communication skills. • Proficiency with Microsoft Office applications and data entry systems. • Ability to manage multiple tasks and work in a fast-paced environment. • Experience in customer service, healthcare, care management, Medicaid, or administrative support preferred.
