📋 Description

• Utilize critical thinking and judgment to collaborate and inform the case management process • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Collaborate with members, providers, and community organizations • Address members’ health care and social determinant needs • Travel up to 75% throughout Metro East/Southern Illinois and surrounding counties • Work independently from home while connecting with teams virtually

🎯 Requirements

• Must reside in the state of Illinois • Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around Metro East/Southern Illinois • Minimum 3-5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Effective computer skills including navigating multiple systems and keyboarding • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted • Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture • Ability to use multiple monitors and applications simultaneously • Ability to use digital communication tools and virtual meetings • Experience with Microsoft Office 365 or similar Google Workspace • Ability to access secure systems such as VPN and EHR portals and follow security practices • Ability to resolve common technical issues independently • Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience • Preferred: Certified Case Manager • Bilingual preferred
