📋 Description

• Collaborate and inform the case management process using critical thinking and judgment • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Serve Medicare and Medicaid dual-eligible members with complex health and social challenges • Collaborate with members, providers, and community organizations • Address members’ health care and social determinant needs • Travel throughout East Central Illinois, including Champaign, Vermilion, Ford, Piatt, Douglas, Coles, and Edgar counties • Work from a designated home virtual work location and collaborate with teams virtually

🎯 Requirements

• Must reside in the state of Illinois • Must possess reliable transportation and be willing and able to travel up to 50-75% of the time from home location in and around East Central Illinois • Minimum 3-5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Effective computer skills including navigating multiple systems and keyboarding • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted • Quiet, secure and private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture • Ability to use multiple monitors and navigate multiple applications • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace • Ability to log in to secure systems such as VPN and EHR portals, maintain security practices, and recognize suspicious emails or links • Ability to resolve common technical issues independently • Openness to learning new skills
