📋 Description

• Collaborate and inform the case management process using critical thinking and judgment • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Collaborate with members, providers, and community organizations • Address members’ health care and social determinant needs across the full continuum of care • Travel throughout the North/Northwest Suburbs of Chicago for field-based work while working from home when not traveling

🎯 Requirements

• Must reside in the state of Illinois • Must possess reliable transportation and be willing and able to travel up to 50-75% of the time from the home location in and around the North/Northwest Suburbs of Chicago • Minimum 3-5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Effective computer skills, including navigating multiple systems and keyboarding • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications • Associate's degree required; bachelor's preferred • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload speeds • WiFi and satellite internet are not permitted • Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture • Experience setting up and using multiple monitors and navigating multiple applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace applications • Ability to log in to secure systems such as VPN and EHR portals, secure devices, manage strong passwords, and recognize suspicious emails or links • Ability to resolve common technical issues independently • Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience • Preferred: Certified Case Manager • Bilingual preferred
