📋 Description

• Assess, plan, implement, and coordinate member care telephonically and/or virtually • Develop individualized care plans and wellness strategies • Identify clinical and social barriers to health outcomes • Connect members with programs, benefits, and community resources • Apply clinical judgment to manage complex health conditions and reduce risk factors • Engage members using communication and motivational interviewing techniques • Work in a queue-like setting • Work Monday-Friday, 9:00 AM-5:30 PM EST • Participate in occasional on-call weekends and holidays on a volunteer basis • Collaborate with members, providers, and care teams to improve outcomes

🎯 Requirements

• Active, unrestricted RN license in your state of residence • Willingness to obtain additional state licenses • 3+ years of acute care RN experience • Associate Degree in nursing • Comfortable working with multiple computer applications and monitors • Compact RN license preferred • BSN preferred • 1+ year of case management experience preferred • Certified Case Manager (CCM) certification preferred • Strong customer service and member engagement skills • Excellent phone-based communication skills • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • WiFi and satellite internet are not permitted • Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines • Ability to work within the state and city where currently residing • Experience with Microsoft Office 365 or similar Google Workspace applications • Ability to use VPN and EHR portals • Ability to manage strong passwords and recognize suspicious emails or links • Ability to troubleshoot common technical issues • Openness to learning new skills • Less than 5% travel for meetings, training, or licensure requirements
