📋 Description

• Collaborate with and inform the case management process using critical thinking and judgment • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Collaborate with members, providers, and community organizations • Address members’ healthcare and social determinant needs • Serve dual eligible populations enrolled in Medicare and Medicaid

🎯 Requirements

• Candidates must live in or near Mt. Prospect or surrounding areas in Illinois • Must reside in the state of Illinois • Must possess reliable transportation • Willing and able to travel up to 75% of the time from home location in and around Mt. Prospect and surrounding areas in Illinois • Minimum 3–5 years clinical practical experience • Confidence working at home and as an independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Effective computer skills, including navigating multiple systems and keyboarding • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, plus proprietary applications • Preferred: 2–3 years Care Management, discharge planning and/or home health care coordination experience • Certified Case Manager preferred • Bilingual preferred • Associate's degree required • Bachelor's degree preferred • Active and unencumbered Registered Nurse license in the state of Illinois required • Dedicated workspace free of interruptions • Dependents must have separate care arrangements during work hours
