📋 Description

• Facilitate the delivery of appropriate benefits and/or healthcare information to determine benefits eligibility • Promote wellness activities • Develop, implement, and support health strategies, tactics, policies, and programs • Ensure delivery of benefits and establish member wellness and successful, timely return to work • Support network management, clinical coverage, and related policies and programs • Perform in-person case management activities within a designated geographic area when directed by leadership or business needs

🎯 Requirements

• Active and unrestricted Minnesota RN license • Registered Nurse with active Minnesota state license in good standing within the region where job duties are performed • Minimum 3–5 years of clinical practical experience preferred, including experience with diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care with Medicare members • Minimum 2–3 years of case management, discharge planning, and/or home health care coordination experience • Ability to occasionally travel within a designated geographic area for in-person case management activities • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Efficient computer skills, including navigating multiple systems and keyboarding • Willingness and ability to obtain multistate RN licenses if needed; company will provide • Certified Case Manager preferred • Associate’s degree required • Bachelor’s degree preferred; applicant would be required to obtain one within 3–5 years as part of role development, subject to state licensing laws
