📋 Description

• Conduct comprehensive telephonic assessments and develop individualized care plans. • Identify health risks, barriers, and opportunities to improve member wellness. • Coordinate care, referrals, and transitions to appropriate clinical programs and resources. • Apply clinical judgment to support members with complex and co-morbid conditions. • Collaborate with multidisciplinary teams to achieve optimal member outcomes. • Maintain compliance with regulatory requirements and company policies.

🎯 Requirements

• Active, unrestricted RN license in state of residence • Willingness to obtain a Compact RN license (company-paid) • Minimum 5 years of clinical RN experience • Associate Degree in Nursing (ADN) • Ability to work within Monday–Friday business hours • 6+ months of Case Management or Utilization Management experience (preferred) • Case Management Certification (CCM or equivalent) (preferred) • BSN (preferred)
