📋 Description

• Complete In-home Evaluations, Focused Visits, and Video Visits • Exemplify Signify Health operational, clinical, and member experience KPIs • Complete up to 80% of the week in visits and reserve one assigned day per week for coaching and mentorship • Provide 40 hours of weekly availability, 128+ capacity units per month, and 88+ completed visits per month • Provide dedicated guidance to new hire clinicians during their first 90 days • Facilitate virtual and in-person shadowing opportunities • Provide best-practice guidance on clinical workflows and diagnostic and preventive services • Offer group support to active clinicians through webinars and office hours • Provide real-time support to in-field clinicians • Serve as the first line of response for clinical inquiries and escalate issues to Clinical Leaders when needed • Test new diagnostic and preventive services and clinical workflow changes and provide feedback • Meet regularly with Clinical Leaders for support and training • Participate in manager, team, clinical, and special-event meetings as needed • Provide rotating evening and weekend mentoring and coaching support • Maintain required supplies and diagnostic equipment • Report to the Operations Manager with a dotted line to the Clinical Success team

🎯 Requirements

• Clinical background and state licensure (e.g., MD/DO, NP/PA) • Minimum 6 months–1+ year(s) of experience as a high-performing Signify Health Clinician • Preferred: multi-state licensure • Preferred: 5–7 years of clinical experience, with a strong background in primary care, chronic disease management, or related field • Experience completing IHEs successfully according to Signify Health operational, clinical, and member experience KPIs • Experience mentoring teams and providing 1:1 professional and personal support • Experience with Signify IHE tools and processes • Experience developing and presenting group presentations • Ability to inspire, motivate, and mentor peers • Ability to share leading practices, tips, and tricks from firsthand experiences • Understanding of managing inquiries and escalating performance issues to leadership • Understanding of upholding high-quality member care across teams • Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies • Ability to provide 40 hours of availability weekly • Ability to travel in-region and locally as dictated by business needs • Ability to accommodate field observations and ridealong experiences • Maintain training, affiliation, and compliance with Video Visit requirements • Maintain training and complete all diagnostic and preventive service products as directed
