📋 Description

• Complete In-Home Evaluations, Focused Visits, and Video Visits • Test new diagnostic and preventive services and clinical workflow changes, providing feedback • Provide 40 hours of weekly availability and maintain 128+ capacity units per month • Reserve one assigned day per week for coaching and mentorship activities • Maintain an 88+ completed-visit-per-month pace • Guide new-hire clinicians during their first 90 days • Facilitate virtual and in-person shadowing opportunities • Share best-practice tips on clinical workflows and diagnostic/preventive services • Provide group support to active clinicians through webinars and office hours • Offer real-time support to in-field clinicians • Respond first to non-urgent clinical inquiries and escalate issues to Clinical Leaders or Operations Managers • Meet regularly with Clinical Leaders for support and training • Participate in manager, team, and clinical meetings • Test and complete diagnostic/preventive service products as directed • Maintain required supplies and diagnostic equipment • Travel in-region and locally as business needs dictate • Accommodate field observations and ride-along experiences

🎯 Requirements

• Clinical background and state licensure, such as MD/DO or NP/PA • Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies • Ability to inspire, motivate, and mentor peers • Ability to share leading practices and practical guidance • Understanding of managing inquiries and escalating performance issues to leadership • Understanding of maintaining high-quality member care across teams • Experience completing In-Home Evaluations according to Signify Health operational, clinical, and member-experience KPIs • Experience mentoring teams and providing 1:1 professional and personal support • Experience with Signify Health In-Home Evaluation tools and processes • Experience developing and presenting group presentations • Preferred multi-state licensure • Preferred 5–7 years of clinical experience, with a strong background in primary care, chronic disease management, or a related field • Preferred minimum 6 months performing Signify Health in-home health evaluations • Maintain training, affiliation, and compliance with Video Visit requirements • Complete all diagnostic and preventive service products as directed • Provide rotating evening and weekend mentoring/coaching support as assigned • Flexibility to travel locally and in-region • Ability to accommodate field observations and ride-along experiences
