📋 Description

• Telephonically outreach to Medicare population to improve medication adherence and/or HEDIS diabetes and blood pressure control • Provide education and address barriers to adherence by utilizing the member's benefits • Ensure member engagement and discern health needs based on key questions and conversations • Help member actively and knowledgably participate with their provider in healthcare decisions • Develop a proactive course of action to address issues presented to enhance health outcomes

🎯 Requirements

• Residence within the Eastern or Central Time Zone • 3-5 years of clinical or practice experience in Renal, Diabetes, Hypertension, Primary Care, Geriatrics, or Oncology • Experience supporting Managed Care and Medicare populations • 1-3 years of experience using Microsoft Office applications • Reliable hardwired internet connection • Active healthcare licensure or certification such as RN, LPN, MSW, LCSW, or CDE are highly preferred • 5 years of clinical or practice experience in Renal, Diabetes, Hypertension, Primary Care, Geriatrics, or Oncology. • Experience with telephonic coaching, patient education, or case management. • Strong computer proficiency, including Microsoft Office. • Ability to work independently with strong time management and organizational skills • Excellent analytical, problem solving, and critical thinking abilities. • Strong communication and interpersonal skills with the ability to build rapport and navigate complex situations; bilingual skills are a plus. • Demonstrated ability to support positive behavioral change using effective communication techniques. • Experience with motivational interviewing and active listening.
