• Provide In-Home Health Evaluations • Engage with patients in their homes • Build trusted relationships to improve health outcomes • Conduct virtual visits as needed • Provide all Signify Health services as indicated including Diagnostic & Preventive Services products
🎯 Requirements
• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • Able to cover the following primary locations: Houghton (Counties: Gogebic, Houghton, Keweenaw, Ontonagon)
