📋 Description

• Provide in-home health evaluations in members’ residences • Engage with members and build trusted relationships • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and within licensed states, potentially up to 100% of the time • Visit counties across the state and other areas based on business needs • Help connect members to appropriate care and bridge gaps in care • Answer health questions and provide health education • Work with Signify Health as a clinician colleague

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Nurse Practitioner or Physician Assistant qualification • Able to travel locally and within licensed states • Able to travel up to 100% of the time across a defined coverage area • Able to stay overnight in hotels and use car rentals and/or flights as needed • Comfortable and safely able to perform duties in homes containing household pets, secondhand smoke, and airborne allergens or irritants • Anticipated weekly hours: 40
