📋 Description

• Provide in-home health evaluations in members’ residences • Travel up to 100% of the time across a defined coverage area, potentially requiring overnight hotel stays, car rentals, and/or flights • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Cover all counties within Iowa and flex to other areas based on business needs • Work as a clinician colleague with Signify Health

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Willingness and ability to travel locally and within licensed states, including up to 100% travel • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Anticipated availability of 40 hours per week
