📋 Description

• Provide In-Home Health Evaluations in members’ private residences • Engage members, answer health questions, and build trusted relationships • Review medications and medical histories • Perform physical evaluations • Provide additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screenings • Help identify connections to appropriate care and bridge gaps in care

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to conduct in-home visits in residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and intuitive clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and diagnostic and preventive services as indicated
