📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with members and build trusted relationships • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states, covering all counties within the state and other areas as needed • Help identify connections to appropriate care and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights
