📋 Description

• Provide in-home health evaluations for health plan members • Engage with members in their homes and build trusted relationships • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Educate members and answer their health questions

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Able to cover the specified primary locations • Able to travel locally and within licensed states, as needed • Able to conduct virtual visits as needed • Able to safely perform duties in private residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Availability for 40 hours per week
