📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with members and build trusted relationships • Review medication and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services products • Conduct virtual visits as needed • Travel locally and within licensed states, potentially requiring overnight hotel stays, car rentals, and/or flights

🎯 Requirements

• Nurse Practitioner or Physician Assistant qualification • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to travel locally and within licensed states • Able to travel up to 100% of the time across a defined coverage area • Able to perform duties safely and comfortably in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Ability to conduct virtual visits as needed
