📋 Description

• Provide in-home health evaluations for health plan members • Engage with members in their homes and build trusted relationships • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and within licensed states • Help bridge gaps in care and connect people to appropriate care • Visit people nationwide across defined coverage areas

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to accommodate overnight hotel stays, car rentals, and/or flights as needed • Ability to conduct virtual visits as needed • MD or DO qualification
