📋 Description

• Provide in-home health evaluations in members’ residences • Travel primarily across a defined coverage area, potentially up to 100% of the time • Conduct approximately 45-minute visits and build trusted relationships with members • Review medications and medical histories • Perform physical evaluations • Provide additional diagnostic and preventive services, including diabetic eye exams, spirometry and bone density screening • Conduct virtual visits as needed • Visit people nationwide to connect them with appropriate care • Refer members to their health plans and encourage participation of loved ones when appropriate • Communicate members’ health and quality-of-life information to their primary care physicians • Use an iPad and clinical workflow for each evaluation • Travel within licensed states, including possible overnight hotel stays, car rentals and flights

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to safely and comfortably perform duties in homes containing household pets, secondhand smoke and potential airborne allergens or irritants • Willingness and ability to travel locally and within licensed states, potentially up to 100% of the time • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to complete medication and medical history reviews, physical evaluations and ordered diagnostic tests • 40 anticipated weekly hours
