📋 Description

• Provide in-home health evaluations in members’ private residences • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Provide Diagnostic & Preventive Services, including spirometry, DEE, and bone density screening • Make referrals to members’ health plans and encourage involvement of loved ones • Ensure primary care physicians are aware of members’ health and quality of life • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Visit people nationwide to connect them with appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • MD or DO qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
