📋 Description

• Provide part-time in-home health evaluations to health plan members • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Provide referrals to health plans and encourage participation of loved ones • Ensure primary care physicians are aware of patients’ health and quality of life • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • MD or DO qualification
