• Provide In-Home Health Evaluations, engaging with people in their homes • Conduct virtual visits as needed • Bring your heart into every visit, joining a national network of purpose-driven clinicians • Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred)
