📋 Description

• Provide Full Time In-Home Health Evaluations in members’ homes • Engage one-on-one with people and help bridge gaps in care • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and intuitive clinical workflow for each evaluation • Visit people nationwide and connect them to appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • Ability to travel locally and within licensed states, as needed • Ability to conduct in-home visits in private residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Ability to safely and comfortably perform physician duties in diverse home environments • MD or DO qualification implied by the required & preferred qualifications section title, but no explicit educational credential requirement stated
