📋 Description

• Provide In-Home Health Evaluations in members’ homes • Travel primarily, up to 100% of the time, across a defined coverage area • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Build trusted relationships and help improve health outcomes • Travel may involve overnight hotel stays, car rentals, and/or flights • Work as a clinician colleague fulfilling the specified role requirements

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Nurse Practitioner or Physician Assistant qualification • Full-time availability, 40 anticipated weekly hours
