📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people and build trusted relationships to improve health outcomes • Review medication and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide and connect them to appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states, with possible overnight hotel stays, car rentals and/or flights

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states, including up to 100% travel • Multiple state licenses as dictated by business needs
