📋 Description

• Provide in-home health evaluations in members’ homes • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medication and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel up to 100% across a defined coverage area, including possible overnight hotel stays, car rentals and flights

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Comfortable and able to safely perform duties in homes that may contain household pets, secondhand smoke and potential airborne allergens or irritants • Able to travel locally and within licensed states • Nurse Practitioner or Physician Assistant qualification • Available for 40 anticipated weekly hours
