📋 Description

• Provide in-home health evaluations in members’ homes • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams and bone density screening • Travel locally and across licensed states within the defined coverage area • Visit people nationwide to connect them with appropriate care • Build trusted relationships and improve health outcomes

🎯 Requirements

• Nurse Practitioner or Physician Assistant clinician • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to comfortably and safely perform duties in homes containing household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Willingness to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Available for 40 hours per week
