📋 Description

• Provide In-Home Health Evaluations in members’ homes • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the person’s health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide and connect them to appropriate care • Travel locally and within licensed states, covering all counties within the state and other areas as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, DEE and bone density screening • Work primarily in private residences of health plan members

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Comfortable and able to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Nurse Practitioner or Physician Assistant qualification • Ability to conduct virtual visits as needed
