📋 Description

• Provide In-Home Health Evaluations in members’ homes • Travel primarily across a defined coverage area, potentially up to 100% of the time • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medication and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and intuitive clinical workflow for each evaluation • Avoid prescribing medicine, ordering lab tests, or altering current treatment regimens • Visit people nationwide and connect them to appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services products such as spirometry, DEE, and bone density screening • Conduct virtual visits as needed • Work primarily in private residences of health plan members

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Nurse Practitioner or Physician Assistant qualification • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willing and able to travel locally and within licensed states, potentially up to 100% of the time • Able to accommodate overnight hotel stays, car rentals and/or flights as required • Able to conduct virtual visits as needed
