📋 Description

• Provide in-home health evaluations in members’ residences • Travel primarily, up to 100% of the time, across a defined coverage area • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, DEE, and bone density screening • Build trusted relationships and help improve health outcomes • Make referrals to health plans and encourage participation of members’ loved ones • Remain flexible for local and multi-state travel

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to travel locally and within licensed states • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Anticipated weekly hours: 40
