📋 Description

• Provide In-Home Health Evaluations in members’ homes • Travel primarily up to 100% of the time across a defined coverage area • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Be flexible to travel locally and within licensed states • Build trusted relationships and help bridge gaps in care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Physician (MD or DO) qualification • Ability to travel locally and within licensed states • Ability to conduct in-home evaluations and virtual visits • Ability to work 40 hours per week
