📋 Description

• Provide in-home health evaluations in members’ residences • Engage one-on-one with members and answer health questions • Review medications and medical histories • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Travel locally and within licensed states • Build trusted relationships and help improve health outcomes • Make referrals to members’ health plans and encourage participation of loved ones when appropriate • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Physician qualification as an MD or DO • Ability to travel locally and within licensed states • Ability to comfortably and safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Ability to conduct virtual visits as needed • Ability to use an iPad and intuitive clinical workflow • Ability to travel up to 100% of the time, including potential overnight hotel stays, car rentals, and/or flights • Multiple state licenses as required by business needs
