📋 Description

• Provide in-home health evaluations in members’ private residences • Engage with members and build trusted relationships to improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical histories • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services • Make referrals to members’ health plans and encourage participation of loved ones • Ensure primary care physicians are aware of members’ health and quality of life • Travel locally and within licensed states, including potential overnight hotel stays, car rentals, and flights • Join a national network of clinicians serving people nationwide

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Flexibility to travel locally and within licensed states • Availability for primarily traveling, up to 100% of the time • Ability to conduct virtual visits as needed • MD or DO qualification stated in the required and preferred qualifications • 40 anticipated weekly hours
