📋 Description

• Provide in-home health evaluations in members’ homes • Travel primarily across a defined coverage area, potentially up to 100% of the time • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Make referrals to members’ health plans and communicate health information to primary care physicians • Build trusted relationships and help bridge gaps in care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willing and able to travel locally and within licensed states, potentially up to 100% of the time • Anticipated weekly hours: 40
