📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage one-on-one with members and answer health questions • Review medications and medical histories • Perform physical evaluations • Conduct additional diagnostic and preventive tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Conduct virtual visits as needed • Provide all indicated Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states across a defined coverage area • Visit members nationwide to help connect them to appropriate care • Build trusted relationships and help improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willingness and ability to travel locally and within licensed states, up to 100% of the time • Ability to conduct in-home and virtual health evaluations • Ability to use an iPad and clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and diagnostic tests as indicated
