📋 Description

• Provide in-home health evaluations in members’ residences • Engage one-on-one with people and answer health questions during approximately 45-minute visits • Review medications and medical histories • Conduct physical evaluations • Perform additional diagnostic and preventive tests when ordered, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services and Diagnostic & Preventive Services products • Travel locally and within licensed states, potentially requiring overnight hotel stays, car rentals, and/or flights • Build trusted relationships and help bridge gaps in care • Make referrals to health plans and encourage participation of members’ loved ones • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Ability to safely and comfortably perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Willingness and ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to conduct virtual visits as needed • Ability to use an iPad and intuitive clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and additional diagnostic tests when ordered
