📋 Description

• Provide in-home health evaluations in members’ homes • Engage with people one-on-one and help bridge gaps in care • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states across a defined coverage area • Build trusted relationships and help connect people to appropriate care • Work as a clinician colleague with Signify Health • Visit people nationwide to support improved health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Ability to travel locally and within licensed states • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals, and/or flights • Ability to work 40 hours per week
