📋 Description

• Provide in-home health evaluations in members’ homes • Travel primarily across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams and spirometry • Use an iPad and intuitive clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states • Meet health plan members in their private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willingness and ability to travel locally and within licensed states, up to 100% of the time • Ability to conduct virtual visits as needed • Anticipated weekly hours: 40
