📋 Description

• Provide in-home health evaluations in members’ private residences • Travel primarily across a defined coverage area, as dictated by business needs • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits with one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic and preventive services such as diabetic eye exams, spirometry, and bone density screening when ordered • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services and Diagnostic & Preventive Services products • Travel locally and within licensed states • Help connect members to appropriate care and make referrals to health plans • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Ability to travel locally and within licensed states • Ability to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Ability to conduct in-home health evaluations, including medication and medical history review, physical evaluation, and diagnostic tests as applicable • Ability to use an iPad and clinical workflow • Ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals, and/or flights
