📋 Description

• Provide in-home health evaluations in members’ residences • Engage with people one-on-one and answer health questions • Review medications and medical histories • Perform physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states, up to 100% of the time • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes • Make referrals to health plans and encourage participation of members’ loved ones when appropriate • Ensure primary care physicians are aware of members’ health and quality of life • Complete visits that may require overnight hotel stays, car rentals and/or flights

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within Virginia and flex to other areas (states) of business need • MD or DO qualification • Ability to travel locally and within licensed states, up to 100% of the time • Ability to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Primary residency in the state of Virginia • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to complete in-home health evaluations and associated visits
