📋 Description

• Provide in-home health evaluations for health plan members • Engage with members and build trusted relationships • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services • Travel locally and within licensed states as needed • Help bridge gaps in care and connect members to appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and intuitive clinical workflow • Nurse Practitioner or Physician Assistant qualification
