📋 Description

• Provide in-home health evaluations to health plan members • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services products • Conduct virtual visits as needed • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Able to cover Aroostook and surrounding area • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
