📋 Description

• Provide In-Home Health Evaluations in members’ private residences • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and intuitive clinical workflow for each evaluation • Engage members one-on-one, answer health questions, and help bridge gaps in care • Visit members nationwide and connect them to appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Conduct virtual visits as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Ability to comfortably and safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Nurse Practitioner or Physician Assistant qualification
