📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Conduct virtual visits as needed • Travel locally and within licensed states as needed

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) Spokane, WA • Board certification required • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to work safely and comfortably in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and intuitive clinical workflow • Ability to conduct virtual visits as needed
